Primary care quality risk
- Star Ratings adherence: MAD, MAH, MAC. The three triple weighted Part D adherence measures, diabetes, blood pressure, and cholesterol. A lapse on any one pulls the score down for every plan that patient belongs to.
- Statin use in persons with diabetes (SUPD, MIPS #438). Diabetic patients aged 40 to 75 are expected to be on a statin. Fairview's SUPD score for the closed 2025 measurement year is 96.3%, 52 of 54 patients, five stars. The patients in the remaining gap have no statin on file, which is a prescribing decision we cannot close from behind the counter, and we send prescribers the list of shared patients with no statin on file on request.
- MIPS quality and cost, your full panel. Non adherent patients are associated with more strokes, infarcts, and admissions, which raise your MIPS cost score and erode the quality measures tied to your reimbursement. MIPS adjustments can reach up to 9 percent, a maximum.
