You almost never find out whether the referral worked. We will tell you.
You arrange the plan, hand it off, and it disappears. Fairview fills the same day, delivers to the home, checks the price before anyone is surprised by it, and then calls you back to say whether the patient actually got their medication. Five star rated for medication adherence, 93rd percentile nationally.

The part of the job nobody writes about
The plan is good. The hand off is where it breaks.
You worked out transportation. You confirmed the follow up appointment. You explained the medications to a daughter who was half listening because her mother was being wheeled to the car.
Then the patient goes home, and you never learn what happened next.
Maybe the prescription was too expensive and nobody said so. Maybe there was no ride to the pharmacy. Maybe the prior authorization took four days. Maybe the bottle sat on the counter because seven new medications arrived at once and nobody could make sense of them.
You find out when they come back through the door, or you never find out at all.
Six things that close the loop
We treat the hand off as our job, not yours.
We fill the same day
Send it before discharge and in most cases it is ready that day. Tell us the timing you need and we will tell you honestly whether we can hit it.
We deliver to the home
Free local delivery. In many cases the medication is at the house before the patient is. No pharmacy trip on day one, which is the day it is least likely to happen.
We check the price before it becomes a problem
Tell us the medication and we will tell you what the patient will actually pay, including our cash price if they are uninsured or the coverage is bad. Better to find out at the desk than at the counter.
We handle the prior authorization
We work it from our end so it is not sitting in your queue or delaying a discharge.
We package by day and time when the regimen is complicated
Adherence packaging, what most families call pill packs. Sealed pouches marked with the day and hour and an icon showing what each medication is for. It is associated with patients taking more of their prescribed doses, and it is most useful in exactly the situations you deal with: many medications, memory changes, a family managing it, or the week after a hospital stay.
We call you back
This is the one nobody else does. Tell us you want to know, and we will tell you whether the patient picked up, whether we delivered, whether there was a cost problem, and whether anything looked wrong on the list.
What we are actually asking for
We are not asking you to steer anyone.
You are required to respect patient choice and to give people options. We know that, and we would not ask you to do otherwise.
What we are asking is to be on the list, and to be the name you think of when someone is going home with nine medications, no transportation, and a family who is already overwhelmed.
If we are not a good fit for a particular patient, tell us and we will say so. We would rather be the right answer sometimes than the default answer always.
Measured, not claimed
93rd percentile nationally on medication adherence.
Our performance is measured independently through EQuIPP, the pharmacy quality platform health plans use nationwide. We have held a 5 star rating for 4 consecutive years.
| Measure | Score | Patients |
|---|---|---|
| Blood pressure medication adherence | 98.2% | 215 of 219 patients |
| Cholesterol medication adherence | 97.2% | 246 of 253 patients |
| Statin use in diabetes | 96.3% | 52 of 54 patients |
| Diabetes medication adherence | 95.6% | 108 of 113 patients |
EQuIPP performance metrics as of Quarter 4, 2025, Medicare Part D population, benchmarked against national pharmacy averages.
These measures describe prescribing and dispensing patterns. Neither is a measure of health outcomes.
In plain terms: when we take a patient, they tend to stay on their medication. That is the outcome your discharge plan was built to produce.
The situations you call us about
Send us the hard ones.
Going home tomorrow with a new medication list
Send it now. We reconcile against the prior list, flag discrepancies, and have it at the house on discharge day.
No transportation and no family nearby
Delivery solves this completely inside our service area. Ask us where we cover.
The medication is unaffordable and the patient will not say so
Tell us before discharge. We will price it, tell you the cash price, and tell you if a therapeutic alternative is worth a call to the prescriber.
The family cannot manage the regimen
Packaging plus one written schedule, in type large enough to read without glasses.
Aides or unlicensed staff are in the home
Medication arrives already dispensed, sealed, dated, and labeled, so nobody in the home is filling a planner. Worth knowing, because families routinely assume aides can do more with medication than they are permitted to.
Something in the medicine cabinet worries you
Send the list or the photograph. A pharmacist will review the whole thing and put findings in writing, at no cost, whether or not the patient ever fills with us.
Get our direct line.
We will send you a one page reference card with our direct number, what we need to turn a discharge around same day, and our delivery area. No sales call unless you ask for one.
Typical response: Within 1 business day
Common questions
What discharge planners ask us.
How fast can you turn around a discharge?
Same day in most cases. Call us with the timing and we will tell you plainly whether we can make it. We would rather say no than miss it.
Do you deliver, and where?
Yes, free local delivery for every Fairview patient. Call for our current service area, and if a patient is outside it we will tell you straight rather than promising something we cannot do.
What about uninsured patients?
Tell us the medication and we will quote our cash price before the patient is discharged, so nobody finds out at the counter.
Will you really call me back about what happened?
Yes, if you ask us to and the patient has authorized it. Federal privacy rules govern what we can share and with whom, so we will sort out the authorization piece with you rather than around it.
Can you review a medication list for a patient who does not fill with you?
Yes. No charge and no obligation. We do it because it is the right use of a pharmacist.
Do you handle prior authorizations?
Yes, for prescriptions we fill.
Can you train our aides or direct care staff?
Yes. We run a free supplemental education session on medication basics for direct care staff. Details on request. It is supplemental and does not satisfy any required training your organization must provide.
Are you going to ask me to steer patients to you?
No. We are asking to be on the list.

The nine medication patient. The one with no ride. The one whose daughter is doing this from Texas. The one you are not sure will make it two weeks.
