What the supplement aisle looks like to a pharmacist

I have been in the supplement and OTC aisle for 13 years as a working pharmacist. The patterns are the same in every store. Thousands of products. Most with weak evidence. Many with proprietary blends that do not specify ingredient amounts. Several with marketing that runs far ahead of the science. Patients spend serious money on products that often do not produce the outcomes the box promises.

The frustrating part is that there are real products in real categories with real evidence. Vitamin D for adults at risk of deficiency. Magnesium glycinate for sleep and muscle cramps. Omega 3s for cardiovascular support. Probiotics for specific situations. Targeted supplements for documented deficiencies. These work. But they get buried alongside the products that do not.

Patients deserve a curated set of products selected by a pharmacist with evidence as the filter, not a wall of options where the loudest marketing wins.

Why a pharmacist should be the curator

Pharmacists spend 6 years studying drug action, interactions, evidence evaluation, and patient outcomes. We are trained to read the studies, to identify what works for whom, to recognize when an ingredient at a particular dose has clinical effect versus when it has marketing. That training is most useful when it is applied to the products patients actually buy.

Wellness+ is curated through that training. Each product gets selected based on evidence, dose, formulation, and quality. Products without credible evidence do not make it in, regardless of how well they would sell. Products with extensive proprietary blends that hide ingredient amounts do not make it in. Products that mostly exist to ride a marketing trend do not make it in.

Why we ship nationally

Independent pharmacy economics are tight. Many independent pharmacies survive on volume of prescription dispensing alone, which is exactly the part of the business PBMs have squeezed hardest. A national product line gives Fairview a revenue stream outside the PBM controlled channels, which protects our ability to keep doing the local prescription work the right way.

The patient buying a vitamin in Seattle is not pulling resources from the patient in Hattiesburg. The opposite. The national line is part of what funds our staffing, our hours, our delivery, our consultations, our med sync, and the time we get to spend with patients who walk through our door on Katie Avenue. The two parts of the business support each other.

How we choose what makes it in

Every product on the Wellness+ line passes the same set of questions.

  • Is there credible evidence the active ingredient does what the category claims?
  • Is the dose at a level that has clinical effect, not a marketing dose?
  • Is the formulation appropriate for absorption and tolerance?
  • Is the manufacturer reputable with quality controls?
  • Is the price fair relative to the cost to source the ingredients?
  • Would I recommend this to a patient walking into Fairview tomorrow morning?

If a product fails any of these, it does not make it onto the line. I would rather have fewer products with clear cases than a bigger catalog full of marginally useful options.

What this lets us do for Mississippi

The national line funds the local pharmacy. The local pharmacy provides the prescription dispensing, the vaccinations, the medication reviews, the deliveries, the statewide service to patients in pharmacy deserts, and the consultations that PBMs do not pay for.

Without the national line, Fairview would be more vulnerable to the same forces that have closed independent pharmacies across Mississippi. With the national line, we can keep operating at the standard we want to operate at. That math matters. The Mississippi patient picking up an antibiotic at our counter is part of the same business as the Texas mother buying her family’s vitamins online. Both are getting served because both are part of the same model.
