There's a reason allergists reach for cetirizine first when a patient says "I tried Claritin and it didn't work."
Cetirizine has a higher H1-receptor binding affinity than loratadine. In plain language: it grabs histamine receptors more aggressively and holds on longer. For people with moderate-to-severe seasonal allergies, the kind that make your eyes swell and your nose pour, cetirizine is often the difference between functional and not.
The trade-off: about 10 percent of people experience mild drowsiness, which is why Dr. Mike recommends taking it in the evening. For most people, that mild sedation has worn off by morning, leaving you with 24 hours of powerful protection.
Cetirizine is my go-to for patients who come back and say that other antihistamine didn't work.
