Here's something most pharmacies won't tell you directly: phenylephrine is less effective than pseudoephedrine (the original Sudafed kept behind the pharmacy counter). Clinical studies show phenylephrine's decongestant effect is modest at the 10mg OTC dose.

I'll be straight with you: phenylephrine is not as strong as pseudoephedrine. The studies are clear on that. But it works for mild-to-moderate sinus pressure and congestion, it's available without showing ID, and it's safe for short-term use. I stock it because my patients need options. If you have significant congestion, especially with a sinus infection, come to my counter and let's talk pseudoephedrine. But if you have mild pressure and just want to get through the day, this does the job. Avoid it if you have high blood pressure or heart disease.
