What chain pharmacies never explain.
Diarrhea is the gut moving too fast. The intestines are designed to move contents forward and absorb water as they go. When that process accelerates abnormally, whether from food poisoning, stress, IBS, traveler's diarrhea, or unknown causes, water absorption is incomplete, and the result is loose, frequent, urgent stool.
Loperamide works by binding to opioid receptors in the gut wall, not in the brain. This is the critical distinction that makes loperamide both effective and safe: it slows intestinal motility and increases the tone of the anal sphincter, which reduces urgency. Because it doesn't cross the blood brain barrier in therapeutic doses, it has no central nervous system effects. No sedation, no mood effects, no addiction risk at recommended doses.
The result: intestinal movement returns to normal speed, water absorption increases, stool consistency normalizes, and urgency resolves, typically within one hour.

Is the generic really the same as the brand?
Yes, by FDA requirement. Generic medications must contain the same active ingredient, at the same dose, in the same form, with the same route of administration as the brand name product. The FDA's bioequivalence standard requires generics to perform within 80 to 125 percent of the brand's absorption profile. Same active ingredient is not a marketing claim. It's a regulatory fact.
