A different right product for heartburn before a triggering meal. Most people don't know there's a difference, and that's why they stay uncomfortable longer than they have to.

What's the difference between Tums, Pepcid, and Prilosec? Aren't they all for heartburn?
They are not the same. Tums (calcium carbonate) is an antacid that neutralizes acid already present. Take it when heartburn is happening. Pepcid (famotidine) is an H2 blocker that prevents acid production. Take it 30 to 60 minutes before a triggering meal or at bedtime for nighttime reflux. Prilosec (omeprazole) is a proton pump inhibitor that suppresses acid for 24 hours. Take it daily for 14 days when heartburn is happening 2 or more days per week. Using the wrong one is like putting an umbrella up after you're already soaked.
Is generic omeprazole really the same as Prilosec OTC?
Yes. The active ingredient (omeprazole 20mg), dose, and FDA approved bioequivalence are identical. The FDA requires generics to perform within 80 to 125 percent of the brand product's blood concentration profile. The price difference reflects what the brand spends on advertising, not what the medication costs to manufacture. Generic omeprazole at $10.65 gives you 28 days of treatment. Prilosec OTC at $15.03 gives you 14 days. Same medicine. Same effect. Better value on the generic.
Why does omeprazole need to be taken before breakfast?
Omeprazole is a prodrug. It must be absorbed, activated, and incorporated into proton pumps that are actively producing acid. Pumps are most active when stimulated by food. Taking omeprazole 30 to 60 minutes before your first meal of the day puts the drug in your system at peak pump activity. Taking it at bedtime or after eating significantly reduces effectiveness. This timing rule is why many patients say omeprazole did not work for them. The timing was wrong.
Is loperamide safe? It's an opioid receptor binder.
Loperamide is safe at recommended doses. It binds peripheral opioid receptors in the gut wall but does not cross the blood brain barrier at normal doses, so it has no central nervous system effects, no sedation, and no addiction risk. It's one of the most effective OTC medications available. Two situations require caution: do not take loperamide if you have a fever above 101 degrees alongside diarrhea (could trap an active infection), or if there is blood in stool (requires medical evaluation, not antidiarrheal medication).
Why does Pepto-Bismol turn my tongue and stool black?
This is completely normal and harmless. The bismuth in Pepto-Bismol reacts with trace sulfur in saliva and stool to form bismuth sulfide, a black compound. It looks alarming the first time you see it but it is not blood and it is not a problem. The color clears within a few days of stopping the medication.
Will fiber make me more or less regular?
Psyllium fiber normalizes stool consistency in both directions, which is unusual for a fiber supplement. For constipation, the psyllium gel adds bulk and moisture to soften stool. For diarrhea, the same gel absorbs excess intestinal water and adds structure to loose stool. This is why psyllium is the only laxative class product recommended for patients with irregular bowel habits (sometimes constipated, sometimes loose). It also has an FDA approved claim for cholesterol reduction at 7 grams daily.
Should I take PEG (Miralax) or a stimulant laxative for constipation?
Start with PEG. PEG (polyethylene glycol) is an osmotic laxative that draws water into the colon to soften stool. It does not cause cramping and does not lead to dependence. The colon does not adapt to it. You can take it daily for months without losing the ability to function without it. Stimulant laxatives (bisacodyl, senna) work faster but can become habit forming with regular use as the gut muscles begin to rely on the stimulus. Gastroenterologists recommend PEG before stimulants for chronic constipation.
