What chain pharmacies never explain.
Diclofenac, the active ingredient in Voltaren, is one of the most studied NSAIDs in medicine. For years it was available only by prescription for good reason: as an oral medication it carries cardiovascular and gastric risk profiles similar to other prescription NSAIDs.
Then researchers discovered something important: when diclofenac is applied topically, less than 10% enters the systemic circulation. The drug concentrates in the local tissue, the knee cartilage, the hand joint, the shoulder, where you need it, with minimal exposure to the stomach, kidneys, and heart.
This is why Voltaren Arthritis Gel was the first topical NSAID approved for OTC sale by the FDA in 2020, specifically for arthritis pain. It is, in effect, a former-prescription anti-inflammatory delivered with the side-effect profile of a topical cream.
For elderly patients, patients with stomach ulcer history, and patients who cannot tolerate oral NSAIDs, Voltaren is often the preferred option rather than just an alternative.

Voltaren is one of the most significant advances in pain management to go OTC in recent years. I recommend it specifically for two patient types: elderly patients with arthritis who cannot safely take oral ibuprofen due to GI or cardiovascular risk, and patients who find that oral NSAIDs aren't providing enough local joint relief. The mistake I see is under-application, most people use a fraction of the recommended dose. Use the dosing card that comes with the tube. Apply 2-4 grams to the affected joint, 4 times daily. And give it one to two weeks of consistent use before judging it, it builds up in the local tissue and works better over time. Don't stop after two days because you don't feel it yet.

How is this different from the brand name on the same shelf?
Generic products contain the same FDA-approved active ingredient at the same dose, in the same form, as the brand-name product. The FDA requires generic medicines to meet bioequivalence standards before approval. The main difference customers notice is the price, not the medicine.

Is this safe during pregnancy or breastfeeding?
OTC pain products vary significantly in pregnancy safety. Acetaminophen is generally considered safe across all trimesters (consult your OB).
