What chain pharmacies never explain.
Diphenhydramine, the active ingredient in Benadryl, is an antihistamine. In oral form, it causes the drowsiness Benadryl is famous for because it crosses the blood brain barrier. In topical form, applied to the skin, diphenhydramine blocks histamine H1 receptors in the skin's sensory nerve fibers, reducing the itch signal without significant systemic absorption. The result: localized itch relief without sedation, provided the product is used correctly on limited skin surface area.
The critical drug interaction Dr. Mike wants on this page: do NOT use topical Benadryl cream at the same time as oral Benadryl (diphenhydramine tablets or liquid). When diphenhydramine is absorbed through skin over a large area, it can add to the systemic levels of an oral dose, increasing sedation, confusion, and anticholinergic effects (dry mouth, urinary retention, rapid heartbeat). The risk is particularly significant in children, elderly patients, and anyone who has taken oral diphenhydramine within 4 to 6 hours.
Apply topical Benadryl cream to small, localized areas (individual bug bites, a small rash patch), not over large skin surfaces.

Benadryl cream is excellent for what it's designed for: a few mosquito bites, a small contact rash, a mild bee sting reaction. Two rules I enforce. One, do not use it at the same time as oral Benadryl. The combined diphenhydramine exposure can cause significant sedation and other anticholinergic effects, especially in children and older adults. Two, do not apply over large skin surface areas. If the itching covers a large area, oral antihistamine is the better choice.

Unopened products in their original sealed packaging can be returned within 30 days of delivery for a refund. For safety reasons, opened skincare and wound care products are not returnable. If something arrives damaged or incorrect, contact us within 7 days and we'll make it right.
Can I use this with my prescription medications or topical Rx?
Most OTC skin and wound products are compatible with prescription topicals, but the order and timing of application matter. Is this safe during pregnancy or breastfeeding?
Most products in this collection are safe in pregnancy and breastfeeding when used as directed on small skin surface areas. Exceptions include hydrocortisone over large areas, terbinafine (Lamisil) during pregnancy, and selenium sulfide (Selsun Blue) on the scalp.
