How to use

The protocol.

  • Apply a thin layer to the affected area and 1 inch beyond the visible border twice daily

  • Treat footwear simultaneously. Use antifungal powder in shoes. Wash socks in hot water.

The real story behind this product

What chain pharmacies never explain.

Athlete's foot (tinea pedis), jock itch (tinea cruris), and ringworm (tinea corporis) are all caused by the same category of organisms: dermatophyte fungi that infect the outer skin layer and feed on keratin, the protein in skin and nails.

Among OTC antifungals, two mechanisms exist. Azoles (clotrimazole, miconazole) inhibit an enzyme in the fungal cell membrane synthesis pathway, weakening the fungal cell wall over time. They are fungistatic, meaning they stop fungal growth but rely on the immune system to clear the weakened organisms. Allylamines (terbinafine, this product) target a different enzyme with a more complete mechanism, accumulating toxic squalene in the fungal cell, which is directly lethal. Terbinafine is fungicidal.

The clinical implication: fungicidal agents achieve clinical cure faster than fungistatic agents. In head to head trials, terbinafine achieves mycological cure of athlete's foot in 1 to 2 weeks. Clotrimazole requires 4 weeks for comparable results. This is why Dr. Mike recommends Lamisil AT over clotrimazole based products when cost is not the primary constraint.

Mississippi context: the heat and humidity create ideal conditions for dermatophyte growth. Warm, moist environments in shoes, athletic gear, and shared spaces drive fungal infection rates higher than the national average.

Terbinafine is my first line recommendation for athlete's foot, jock itch, and ringworm. The fungicidal mechanism means faster real world cure rates than the azoles most people reach for. The critical thing I tell every patient: complete the full treatment course even after symptoms disappear. Symptoms resolve before the fungal infection is fully cleared. Stopping early leaves residual fungal cells that will repopulate the area. For athlete's foot: apply twice daily for 1 week, then continue for 1 additional week after symptoms clear.
